- Providers can use the LS4H referral form to sign up patients for the program. Please make sure to send the completed form to us for processing as soon as the patient is finished.
- Providers can download promotional LS4H Pocket cards to hand out to patients who might profit from the Little Steps 4 Health initiative. These advertising cards are useful tools for introducing patients to the program and enticing them to participate.
