Faculty Performance Appraisal (Faculty Annual Evaluation)
Faculty Performance Appraisals (Faculty Annual Evaluations) are completed annually by employed Faculty. The goal of the Annual Faculty Performance Review process is to provide a meaningful assessment of faculty achievements, promote the development of goals and expectations, foster and support faculty development and mentorship, and guide junior faculty in career advancement through formative discussions with the Department Chair/Division Chief.
In order to standardize, streamline and most importantly integrate the annual faculty evaluation process with promotion advancement, the Human Resources Office has developed a web-based faculty activity and evaluation system. After responding to few general questions which are part of the evaluation process, the system will direct faculty to this webpage to access the Activity Reporting Template. The template is available in either EXCEL and WORD. Only one template needs to be completed. Please select the format you are most comfortable in completing.
