EVMS offers employees a choice of several health plans, a dental, and vision plan. You may include your family under the policy by contributing the required cost of such additional coverage. It shall be your responsibility to request and obtain the coverage. Premiums may be paid on a pre-tax basis or post-tax basis.
Employees who elect to use pre-tax dollars cannot drop or make changes to the plan until the annual open enrollment period unless an approved status change occurs (i.e., marriage, divorce, new child). Employees who elect post-tax dollars cannot change plans until open enrollment; however, they may cancel coverage at any time.
Employees who commence employment on or before the 15th of the month will be effective on health and/or dental insurance on the first of the following month. Employees who commence employment after the 15th of the month will be effective on the health and/or dental insurance the first of the second month: i.e., if hired April 16th coverage is effective June 1st.

Open Enrollment begins Monday May 6th, 2013 and continues through Friday, May 17th, 2013.
During this period, all benefit eligible employees may enroll, waive or make changes to their EVMS benefits. Changes made during the Open Enrollment period will take effect on July 1, 2013.
There is no automatic enrollment for the Health Care Reimbursement Account and the Dependent Care Reimbursement Account. You must actively make an election to enroll every year.
