Studies Involving Sentara Sites

If  you will be conducting your study at any of the Sentara locations and/or accessing any Sentara medical records you must obtain approval from Sentara.  Use this link to complete a Research Request Form and to read about your responsibilities as an investigator involving Sentara facilities and/or data.  

To request access to Sentara data:

1. Complete the VHS Data Request Intake Form.  
2. Request data through SENTARA Research Data Request Portal.
3. Complete and upload a  SENTARA Data Use Attestation.
4. For projects involving any trainees, there must be a VHS faculty mentor as the PI.

For projects involving medical students, the project must be cleared through Medical Student Research
