EVMS Medical Group recently achieved a top-tier designation from the National Rural Accountable Care Consortium. Only the top 15 percent of enrolled practices receive the Exemplary Practice designation.

Selection is based on engagement in the Practice Transformation Network program, improved quality performance on health outcomes and overall healthcare in the community.

The medical group monitored metrics for Medicare wellness exams and screenings for falls, depression, mammograms and colorectal issues.

“We saw improvements in every single category,” says Allison Gray, RN, EVMS Medical Group Assistant Quality Officer. “The work that has gone into place for additional screening for staff has been a large undertaking, but the staff has done this with a positive attitude and recognizes the quality and value it will add." 

EVMS Medical Group joined the Practice Transformation Network in 2018 to help prepare for becoming an Accountable Care Organization (ACO) in the future. An ACO, according to the Centers for Medicare and Medicaid Services, is an organization of healthcare practitioners that agrees to be accountable for the quality, cost and overall care of the Medicare patients they serve.

“We wouldn’t have been able to achieve this designation without the hard work and effort from our primary-care and specialty-care practices,” says Ms. Gray. “They committed to monthly meetings, reviewing reports and implementing screenings. This is all thanks to them.”

EVMS Medical Group will also be recognized by the Center of Medicare and Medicaid Services for this honor.
