Requesting a Duplicate Diploma

In accordance with the Family Education Rights and Privacy Act (FERPA), only the individual earning the diploma can request it.

The Eastern Virginia Medical School typically issues only one diploma to each graduate of EVMS.

The signatures of EVMS officials may not be the same as those on the original diploma since the signatures of former officials are not maintained on file. As a result, diplomas are reissued in the current format and bear the signatures of current EVMS officers, no exceptions.

In case of loss, theft, or destruction of an original Eastern Virginia Medical School diploma, replacement diplomas can be obtained by filing a request with the Office of the Registrar. To request a replacement send a letter stating the reason for the replacement and a check or money order in the amount of $30US made payable to Eastern Virginia Medical School. Requests should be sent to:

Preparation of the document may take four to six weeks. When the diploma is ready, it will be sent via insured and certified mail. Letters of request must include the following information:

  • Full Name
  • Diploma Name (name recorded on the original diploma)
  • Degree Awarded
  • Date the degree was granted
  • Original Signature
  • Shipping address including a telephone number or e-mail address

Please note:

  1. Damaged diplomas MUST be returned in order to process the request.
  2. If a diploma is destroyed in fire or any other catastrophic event, we require a copy of the insurance or policy report.
  3. If a diploma was stolen, we require a copy of the insurance or policy report.
  4. If the diploma is lost in a move, we require some type of proof be it insurance or sworn/notarized statement stating the facts.
  5. Legal Name Changes: Original diploma must be returned, along with documentation (copy of driver's license, court order with name change)
  6. In cases where additional diplomas are required for professional practice, the holder must submit a notarized declaration of the requirement for each diploma and of the address at which the diploma will be displayed.

NOTE: All diplomas will have re-issued printed in the right hand corner with the date it was re-issued.
