2007/08 Application Process for

Biotechnology, Embryology, Public Health, and Surgical Assistant Students

In order to receive financial aid, students must:

• Complete the 2007/08 Free Application for Federal Student Aid (FAFSA) by March 15, 2007.
  • To apply online, you must have a Personal Identification Number (PIN). It takes three days to receive it electronically or seven to ten days to receive it in the U.S. Mail.
  • Enter the EVMS School Code: 010338.
  • DO NOT leave any fields blank.
  • DO NOT use estimates. Complete the 2006 IRS 1040 tax form before completing the FAFSA.
• Submit the 2007/08 EVMS Financial Aid Application by April 30, 2007.
• Submit a SIGNED copy of your 2006 IRS 1040 tax form if you were selected for a process call Verification. If you are married and you and your spouse filed separately, submit SIGNED copies of both tax returns.
• Submit a 2006 Non-Filer Statement  if you have not and will not file 2006 income taxes by April 30, 2007.
• If you are a MPH student, submit the 2007/08 MPH Educational Plan by May 1, 2006.
