Student HealthHealth insurance is MANDATORY for students (see student handbook).
. Coverage is effective August 1st of each academic year and continues thru July 30th. Each year, all full-time registered students are automatically enrolled in the EVMS Student Health Insurance Plan unless proof of comparable coverage is provided. Students who enroll in the United Health Care Plan must see their assigned site before seeking care outside of the student's assigned site. Failure to do so will result in denial of claims. Site assignments are available on the student portal. Those students who wish to waive the Student Health Insurance must do so by completing a Student Health Insurance Waiver. Student Health Waiver Information For those students who will be keeping the student health insurance coverage and may need to see a physician or fill prescriptions you may click on the enrollment link below and complete the enrollment process. Once enrolled visit the United Health Care website and follow the instructions to create a student health insurance account. Once created temporary cards can be printed. Students that wait till the end of the waiver period to be default enrolled will not receive an insurance card until the end of the waiver period. Once enrolled a card will be mailed to the address provided within 48 hours. Student Health Online Enrollment Form At the close of the waiver period a report will be provided to Financial Services and student accounts will be adjusted accordingly. Failure to complete the waiver or if your waiver is denied due to inadequate existing coverage, the fee will stand and you will be covered by the EVMS Injury and Sickness insurance plan.Please note that the spring waiver is for new students only; students who fail to waive in the fall semester will be covered for the entire plan year. Policy highlights include: • $200,000 maximum benefit for each injury or sickness • $250 deductible per Insured Person per Policy Year • $500 deductible per family per Policy Year • 80% of Preferred Allowance for Preferred Providers • 50% of Usual and Customary for Out of Network Providers. • $5,000 Prescription Drug maximum benefit per policy year.
Dependent Coverage: Students may purchase coverage for their dependents by creating an United Health Care Account and following the instructions to enroll dependents. Dependent coverage is purchased directly thru United Health Care and is not billed thru the Student Disbursement Account. Dental and vision discounts: If you are enrolled in the United Health Care Student Health plan, you will be provided with a discount card for dental and vision services, using local area network providers. Even if you are not enrolled in the United Health Care Student Health plan, you may purchase the same discount card for $25 by clicking the United Health Care Student Health plan icon. In addition, United Health Care Student Health is offering the United Health Care Dental PPO Insurance Plan. You can choose to visit a participating or non-participating dentist for care. Enrollment in the Dental Insurance must be done online at the UHC website and paid for at the time of election.
