Insurance & Prior Authorizations
We participate with most insurance plans and will submit all claim forms for you. Any expenses not covered will be collected at the time of service.
Please check with your insurance company about coverage, preauthorization, and any out-of-pocket expenses. If a preauthorization is required, please contact your primary care physician before surgery to obtain one. If a prior authorization has not been obtained prior to the surgery date, your appointment will be rescheduled.
