Health Requirements for Incoming
Residents and Fellows
Incoming residents must have a physical
exam and submit health forms 45 days before the start of residency.
Copies of immunization documentation, laboratory results and chest X-ray
results must accompany the EVMS health forms. In addition, incoming
residents and fellows must schedule and successfully complete a drug
screen during the month of June.
Please note that you must have a
physical examination by a physician, nurse practitioner, or physician
assistant, who is not related to you, within four months prior to the
program start date. This physical must be documented on the attached
Medical Questionnaire and Physical Exam Form. You must also attach
copies of immunization documentation (i.e., shot records) and laboratory
results to the Medical Questionnaire and Physical Exam form. Forms and
supporting documentation should be mailed to the Occupational Health
office in the enclosed blue envelope or in a separate envelope addressed
to:

Health forms will be treated as confidential medical information.
Do not
return the forms to the program you will be entering.
You must complete all requirements and submit this documentation to the
Occupational Health office by May 12. Failure to comply with the
School’s health requirements will result in restriction from academic
and clinical activities. The resident/fellow is responsible for the cost
of the physical exam, tuberculosis tests, and any necessary
immunizations. Hepatitis B vaccine, however, is offered to all incoming
residents/ fellows who have not been previously vaccinated.
In addition, EVMS is a drug- and alcohol-free workplace. Employment
is contingent upon the successful completion of a drug screen. Drug
screens can be scheduled Monday through Friday during the month of June
at the Occupational Health office by calling 757-446-5870 or
757-446-7105. If you are not able to have a drug screen in June, you
will be given an appointment during your orientation period.
The following are pre-matriculation requirements:
- Tuberculosis surveillance:
You are required to provide documentation
of a two-step Mantoux PPD skin test within 4 months prior to the program
start date.
- If you have had a previous positive PPD, attach a copy of a chest
x-ray report done within the past year and documentation regarding
treatment dates with antibiotic therapy or why treatment was
contraindicated. Please contact us to receive a TB Symptom and
Surveillance form that must also be completed.
- Measles, Mumps, Rubella (MMR) immunity:
Attach documentation of two
doses of live MMR vaccine if you were born in or after 1957 or copies of
measles, mumps, and rubella titers indicating immunity.
- Varicella immunity: Attach a copy of a varicella titer indicating
immunity or documentation of two doses of varicella vaccine.
- Polio immunizations: Attach documentation of childhood polio
immunization series.
- Diphtheria/Tetanus/Pertussis:
Attach documentation of DTP/DTaP
series.
- Hepatitis B: You will be offered Hepatitis B vaccine if you have not
previously received this vaccine series. If you have had the vaccine
series, attach documentation of the dates of the three doses. If you
have had an antibody titer done, attach a copy of the titer result
indicating immunity.
- Tetanus/Diphtheria/Pertussis (Tdap): Attach documentation of a Tdap
immunization (if it has been less than two years since your last Td
booster, attach a copy of the Td booster documentation).
