Longitudinal Care Elective

Learning Activities
- Conduct initial history, physical and comprehensive geriatric assessment for new
patients visiting the memory assessment clinic. (problem focused, consultative clinic)
- Interview caregivers for collateral information. Assist neuropsychologist and geriatric
social worker with testing, evaluations, and follow-up.
- Order appropriate tests based on findings and refer for additional consultative
services.
- Develop appropriate care plan including timetables for updates and subsequent
evaluations.
- Participate actively in the multi-disciplinary team to safeguard continuity of care.
- Provide support to caregivers by making home visits, investigating potential resources
for in-home, community or facility-based support, and assisting with application and
eligibility process.
- Maintain patient log containing initial and ongoing assessments, progress notes, and
minutes from interdisciplinary team meetings, reports from family conferences and
contacts, and care plans.
- Research pertinent geriatric topic, with guidance from supervising faculty, formulate
paper and present educational program with suitable handouts and visuals to at least two
audiences. (e.g.: clinic staff, family members, patients, supports groups, other student
groups)
- Work under the supervision of the medical director for the Memory Assessment Clinic.
Evaluation
Evaluation of the geriatric longitudinal care experience will be based on successfully
completing the learning activities.
