Curriculum - Descriptions of Rotations

Ultrasound and Genetics

Ultrasonography training and experience are ongoing throughout the four years of residency; however, focused hands-on experience occurs during the formal ultrasound/genetics rotation.

Structure

The first-year resident participates in the ultrasonography/genetics rotation. The Center for Advanced Fetal Therapy at Hofheimer Hall (EVMS) and Sentara Norfolk General Hospital (SNGH) provides NST, biophysical profile, contraction stress test, Doppler flow studies, amniocentesis and 3-D ultrasound. Faculty and the diagnostic center are AIUM-certified.

The prenatal diagnosis and genetics program is staffed by 13 licensed ultrasonographers, six board-certified or board eligible genetic counselors and five perinatologists who provide ultrasound imaging for prenatal diagnosis of fetal malformation, birth defects and complex gynecologic pathology. The program provides a wide array of services, including prenatal and preconception genetic counseling, detailed ultrasound imaging, first-trimester screening for Down Syndrome, chorionic villus sampling, amniocentesis and parental and fetal DNA and enzyme analysis, when appropriate.

Teaching

The MFM faculty teaches hands-on diagnostics during the ultrasound/ genetics rotation. The department chair, who completed fellowships in maternal-fetal medicine and ultrasound and prenatal diagnosis, leads an annual ultrasound two-day course that residents attend. All ultrasonographers are certified through the American Registry of Diagnostic Medical Sonographers.

First-year residents follow a panel of patients and are involved in counseling patients. Residents read journal articles, which they review with a genetic counseling supervisor.

Residents are trained to counsel patients with hemoglobinopathies, cystic fibrosis, genetic diseases of patients of Jewish decent, issues related with perinatal loss and counseling regarding the more common congenital anomalies, many hereditary disorders and advanced maternal age.

Supervision

A registered diagnostic medical sonographer oversees resident bedside training in gynecology and obstetrics ultrasound. The perinatologists and genetic counselors supervise resident learning and participation on this service.
