Residents receive training in the evaluation and management of pediatric and adult surgical patients. Structure First-, second- and fourth-year residents rotate through the operative gynecology service for a total of 12 months. Residents on this service participate in all surgeries scheduled from the gynecology generalist division and continuity and specialty clinics. Residents also assist in most private surgeries with community faculty. The generalist division of the Department of Obstetrics and Gynecology is the exclusive provider of gynecology services for Children’s Hospital of The King’s Daughters (CHKD). Residents consult on pediatric patients in the emergency room and gain experience in acute gynecology emergencies and gynecologic injury of pediatric patients. Residents participate in a weekly pediatric gynecology clinic at Hofheimer Hall (EVMS). They operate on these pediatric patients at CHKD. Additionally, while on the SNGH gynecology service, residents participate in a twice monthly colposcopy clinic. This clinic is located in Hofheimer Hall (EVMS). Patients are referred from other departments at EVMS, from regional health departments, and nearby correctional facilities. Residents perform (under direct supervision) cervical colposcopy with biopsy, cervical LEEP and vaginal and vulvar colposcopy. The colposcopy clinic serves a large HIV-positive population, as well as obstetrics office patients referred from the academic obstetrics service (AOS). Residents learn advanced operative laparoscopy and operative hysteroscopy, as well. Residents perform inpatient gynecologic consultations on patients in SNGH’s emergency room. This allows for broad experiences in the management of acute gynecologic emergencies, including gynecologic trauma. Residents also provide consultation on in-patients from other specialties (internal medicine, hematology oncology, surgery and cardiology) broadening the resident experience managing gynecology conditions in patients with co-morbid conditions. Teaching SNGH clinic cases are reviewed in a weekly preoperative conference to discuss procedures and preoperative and postoperative medical management. Residents round daily at SNGH with a member of the generalist division. Located at SNGH, the clinical skills simulation laboratory includes a laparoscopic simulator for basic and advanced skill acquisition at graduated intervals; attendings teach residents to remove ectopic pregnancies on the simulator. At DMC, training is conducted one-on-one while working with community faculty. Supervision The gynecology generalists supervise all consultations, colposcopy procedures, rounds and surgical procedures. The chief resident assigns cases to junior residents. During the clinic cases, the chief acts as the principal physician. The attending is scrubbed for all surgeries and acts as an assistant while supervising residents. At DMC, community faculty supervise residents by assisting in surgery and clinics. |
