Educational Plans for Masters of Public Health, Biomedical Research or Biotechnology

Students enrolled in the Masters of Public Health, Biotechnology (new for 2013-2014), or any Biomedical Research programs must file an Education Plan for the academic year.  This form is used to advise the Office of Financial Aid of planned enrollment and delivery mode of class (i.e., in classroom or distance).  Students must update this form throughout the academic year based on any changes to courses, including method of delivery.
