EVMS requires completion of the Immunization Requirement form with supporting documentation of proof of immunity to measles, mumps, rubella, varicella, polio and tetanus/diphtheria/pertussis. In addition, a two-step tuberculin skin test is required. If an accepted candidate has had a previous positive tuberculin skin test, they must provide a chest X-ray report from less than one year prior to the program start date, documentation of evaluation for preventative therapy from the treating physician and a completed Tuberculosis Symptom Surveillance Questionnaire.
Pre-matriculation immunization forms and documentation must be submitted directly to the EVMS Occupational Health Office. Failure to meet pre-matriculation health requirements in a timely manner may preclude a student from matriculating.
