Supplemental Application
Please do not submit the
Supplemental Application to the Office of Admissions until you have been
requested to do so.
This page contains instructions for downloading and using
EVMS' Supplemental Application forms for admission to the medical school.
These documents are in Adobe Acrobat 5.0 Portable Document
Format. The free Adobe Acrobat Reader is required in order to read and print the forms. If
you do not have the reader, or have a version earlier than 5.0, please download it now.
Make a note of the location on your hard drive where the file was saved. After the
download is complete:
- double-click the newly downloaded file
- follow the instructions on your screen to install the
Reader
- return to this page to download and read or print the
forms
You may now download the materials one at a time in PDF
format, or all at once in a single compressed file.
Documents in PDF Format:
The
following forms may be filled out online or offline through your
browser, then printed and mailed to us. IMPORTANT NOTE: Unless
you have Adobe Acrobat (not the Reader) installed on your system,
you must fill out the application completely - you cannot partially
fill it out, save it, then continue later.
To use this compressed file, you will need a program to
extract the individual files from it. For Windows, Winzip
is a good choice; for other platforms, please search for compression software at your
favorite software site. Again, you will need to download and install the compression
software in order to use this compressed file.
